Property Loss Notice Form

  • Non-Employees

    All property loss, property damage, and personal injury of non-employees must be reported to the Business & Finance Director and the Buildings & Grounds Office on the Property Loss Notice form. Complete and specific information is necessary.  Please report within 24 hours.

    School Property

    Incidents of property loss or damage, theft, and vandalism involving school property must be reported to the Police Department and the Buildings & Grounds Office (Property Loss Notice form).   Report upon discovery and notify Business and Finance Director within 24 hours.
